The treatment of hips with abductor muscle dysfunction following failed total hip arthroplasty (THA) presents a challenging problem. This study evaluated the results of circumferential soft tissue release and trochanteric resection in this condition. Between 1986 and 1994, 40 patients (42 hips) had circumferential soft tissue release and trochanteric resection performed. The mean age of the patients at the time of surgery was 62.3 years (range, 38-81 years). A variety of diagnoses were found, including avascular necrosis, slipped capital femoral epiphysis, post-traumatic arthritis, and rheumatoid arthritis. The mean follow-up period was 6.2 years (range, 3.0-15.0 years). There were 30 female and 10 male patients. All patients were evaluated preoperatively and postoperatively with use of Harris hip scores, gait analysis, thigh and groin pain scales, and radiographs. The mean preoperative Harris hip score was 51 (range, 33-77). After surgery, the mean Harris hip score improved to 96 (range, 71-100). All patients except one had significant improvements in gait and pain. The mean flexion improved from 28 degrees to 59 degrees. A new and increased limping gait was noted in only one patient who had preoperative trochanteric pain. There was no evidence of trochanteric nonunion, infection, or sciatic neuropathy in any patient. The most frequent complication was limited abduction. This study confirms the value of circumferential soft tissue release and trochanteric resection for patients with hip abductor dysfunction after failed THA.
